Another one got killed by the subway train, why?

HK and lots of other Asia subways got barrier that only opens when train is already in position and opens RIGHT in front of the lines for waiting passengers to get ready for boarding.  The arriving passengers exit from the middle.  Never any people falling into the subway.

Here TO and NYC for sure do not have any barrier and annual fatalities is like one per month accident or not (suicide).
How hard is it for N. American cities to do what Asian cities already did  ? ? ?

根据加通社消息，昨日上午一名50多或60多岁的亚裔男子在多伦多市中心最繁忙的地铁站被推下月台后死亡。今日，警方公布嫌疑人姓名。

嫌疑人为57岁的John Reszetnik，他已经被指控一级谋杀罪名。今日上午他已经短暂出庭，之后将继续被羁押。

事件发生于多市中心最繁忙的央街夹布尔街（Bloor-Yonge station）地铁站。受害人在昨日上午约10时15分左右，身处地铁2号线东向行驶的月台。当时一辆进站列车刚刚驶过月台的中心点，受害人准备上车时被推后跌落轨道。警方此前将受害者描述为是一个50岁或60岁的亚裔男子，白色头发。（Grace编译）
